Genetic Alterations (FGFR)
Cancer cells typically contain some changes to the DNA that helps them grow out of control. These changes, or more technically “genetic alterations”, hijack the cell’s regulation of growth and division. These alterations may involve making multiple copies of a gene (so called “amplification”), or fusing different unrelated genes together (called “translocations” or “fusions”), or changing the sequence of a gene (called “mutation”). When proteins are then produced based on this altered genetic blueprint, the control of the cell lifecycle is disturbed and the cell can learn survival behaviors that lead to cancer.
One of the proteins often altered in multiple cancer types is a molecule found on the cell surface called the “fibroblast growth factor receptor” (FGFR). When the genes for FGF receptor are altered:
-it helps the cell to grow and divide uncontrollably,
-it leads to the formation of blood vessels to supply nutrients to this newly dividing colony of cells (cancer),
-it helps the cell detach and travel through the circulatory system to another place in the body (resulting in metastases),
-and most importantly it helps those abnormal cells survive.
Gene alterations of FGF receptor- including fusions, mutations, and amplifications – have been found in many tumor types including bladder, cholangiocarcinoma, brain, breast, cervical, colon, endometrial, esophageal, gall bladder, gastric, kidney, lung, prostate and pancreatic cancers as well as cancers of unknown primary origin.
Clinical trials
A ‘clinical trial’ is a research study in which people agree to test a potential new treatment to prevent or improve a disease or medical condition. A clinical trial also looks at how participants react to the potential new treatment and if any unwanted effects occur. This helps to determine if the new investigational treatment works, is safe, and is better than those that are already available. Many clinical trials also compare existing treatments or test new ways to use or combine existing treatments.

The FUZE trial is a Phase II trial in adult male and female patients with advanced or metastatic tumors whose cells show specific FGFR gene alterations, namely FGFR1, FGFR2 or FGFR3 gene fusions. To be eligible, patients must require therapy and have radiologic and/or clinical progression following at least one prior standard treatment or have no satisfactory alternative treatment options. In this trial, biliary tract cancer (such as cholangiocarcinoma), urothelial cancer (such as bladder cancer) and other solid tumors (such as lung cancer, head and neck cancer, breast cancer, endometrial cancer, colon cancer and others) are being studied. Patients with primary brain tumors cannot enter this trial.

Phases
